How does an inmate go about getting a credit report? Where can he get the proper forms to fill out without access to the internet?

Dear NTP,
If a person is incarcerated, they must mail their request for a credit report. Just as any consumer, they are entitled to a free annual credit report. To request a report they should mail their request to:

Along with the letter requesting a report, they need to include a notarized letter from a warden or other prison authority validating the prisoner's name and inmate number. The letter must be on the correctional institution's letterhead.
The credit report request should include include their full name, Social Security number, date of birth, and addresses for the past two years.
